Applicants between the ages of 18-54 must provide proof of disability by submitting a copy of one of the following: -Medicare card, federally issued red, white and blue card; -California DMV Disabled Placard Registration Parking placard receipt; -Regional Transit Connection (RTC) Clipper card; -Proof of certification with East Bay Paratransit;
If an applicant is unable to provide any of the proofs listed above, the applicant will need to complete Section IV of the application. This section authorizes R-Transit to obtain information from a licensed healthcare professional regarding an applicant's disability.
In order to verify an applicant's eligibility, R-Transit will submit a request to the specified medical center in Section IV of the application to complete a Disability Verification form. The licensed healthcare professional will determine temporary or permanent eligibility for R-Transit services. The average turn around time to receive the from back from the medical center is 10 business days. To expedite this process, an applicant may download the Disability Verification form for the licensed healthcare professional to complete and return it with the application.
The completed disability verification form must have a stamp from the medical facility/provider. |
55 - above

Individuals age 55 and above, who meet the residency requirement will qualify for the R-Transit program regardless of mobility challenges. Therefore, applicants age 55 and above are exempt from completing Section IV of the application.
